TRT and HRT: understand the conversation
TRT usually refers to testosterone replacement therapy. HRT means hormone replacement therapy and is often used when discussing menopausal hormone treatment. They are not interchangeable programs, and neither is automatically appropriate because of age or a single symptom.
Our hormone education topics offer a starting point for questions about testosterone, estrogen and progesterone. Use them to prepare for a clinician-led discussion, not to diagnose yourself or choose a medication.
When low testosterone is the question
In men, an evaluation for testosterone deficiency considers compatible symptoms alongside consistently low levels. The Endocrine Society recommends confirming low testosterone with repeat morning fasting testing and assessing the cause. One isolated number is not the whole diagnosis.
Tell the clinician about fertility plans: testosterone treatment can suppress sperm production. Treatment decisions also require a discussion of medical history, potential adverse effects and monitoring. Learn more in the Endocrine Society’s testosterone guidance.
Before your visit, write down what has changed, how long it has been happening and how it affects daily life. Include medicines, supplements, sleep patterns and any previous hormone treatment. The goal is to understand the problem before choosing a response.
When perimenopause or menopause is the question
Hot flashes, night sweats and vaginal discomfort can lead to very different treatment conversations. Systemic hormone therapy and local vaginal therapies have different roles; nonhormonal options may also be relevant. Suitability depends on symptoms, age, medical history and the treatment being considered.
For someone with a uterus, systemic estrogen generally requires a progestogen to protect the uterine lining. Discuss clotting, stroke and cancer risks as they apply to your history and regimen. The Menopause Society’s hormone therapy guide explains why treatment is individualized.
Bring a record of cycle changes, symptoms and any unexpected bleeding. Ask which concerns require further evaluation and which treatment goals are realistic. You do not need to arrive knowing the medical vocabulary.
Your appointment preparation list
- List your top two or three concerns and when they began.
- Bring current prescriptions, supplements and recent test results.
- Share relevant family history, previous surgeries and past reactions to treatment.
- Ask what testing is appropriate and how follow-up would work.
- Discuss costs, options and what happens if treatment is not the right fit.
“Bioidentical” is not a guarantee that a product is safer or better. Ask about the exact formulation and its evidence. A good plan should make sense to you, including its limitations.
Good questions. Clear answers.
Before you book
Can I book without having recent labs?
You can request a consultation and ask what records or testing the clinician wants. Do not assume that every person needs the same hormone panel.
Will hormones fix fatigue or weight changes?
Not necessarily. Those concerns have many possible causes. An assessment can help decide whether hormone evaluation or another approach makes sense.
How much does TRT or HRT cost?
Confirm current consultation, testing, medication and follow-up costs directly with the clinic. The plan and total cost depend on your evaluation; a prescription is not guaranteed.
